POLICE RECORDS REQUEST FORM

All government records of Centerville City are maintained, classified, and subject to access in accordance with the Utah Government Records Access and Management Act (GRAMA), as set forth at Utah Code Ann. §§ 63G-2-101, et seq., as amended. Identification must be provided prior to the release of a record pursuant to Utah Code 63G-2-202(6)

Police Reports (per case number) $10 for first 50 pages (additional pages = $.20/single sided page; $.35/double sided page)

Accident Reports (per case number) $10 for first 50 pages (additional pages = $.20/single sided page; $.35/double sided page)

Audio/Video/Photo

• $10.00 (up to 4GB)
• $25.00 (Up to 8GB)
• $35.00 (Up to 16GB) Hourly Cost of Staff Time for Compiling, Searching, Formatting, Redacting, Etc.
• $45.00 (Up to 32GB) $55.00 (Up to 64GB)
  • Per hour charge at the current hourly wage of the lowest paid employee who has the necessary skill and training to fulfill request.
• Mailing Costs = Actual costs

I would like to receive copies of the records. I understand that I will be responsible for copy costs. I understand that prepayment of copies over $50 may be required and that I will be contacted if estimated costs are greater than the above specified amount.
